First confirm that the operation is handled by the Mohave County Department of Public Health Environmental Health Division. The county’s business resources distinguish county-regulated food establishments from Arizona cottage food operations, which register through the Arizona Department of Health Services and may not need a county food-establishment permit. Cottage food operations can be missing from the county’s inspection rows for that reason.
Start with the month, then match the operation
Open the county’s Food Safety Inspection Reports page and select the month containing the date you are researching. On October 20, 2026, the page listed files from January 2025 through September 2026 (the September 2026 file was the newest). Older county records may exist beyond that online range.
Inside the PDF, use Find for a distinctive name fragment or street number. The August 2026 summary shows five fields to match: Inspection Date, Rating, Establishment, Address, and Description. It groups rows under Kingman & North Strip, Bullhead City, and Lake Havasu City.
- Confirm the area heading and complete address.
- Copy the date, rating, establishment name, and Description exactly.
- Scan that month for another row with the same address or Description.
- If two rows share a name or address, keep them separate until the county confirms the operation.
The summary can list separately described operations at one premises with different ratings, and it shows neither a permit identifier nor a visit purpose. So a later row could be a routine visit, a follow-up or a reopening check; the detailed report says which.
Mohave County’s four ratings, from Excellent to Unsatisfactory
Mohave County’s food-safety FAQ defines four ratings:
| Rating | What the county’s label indicates |
|---|---|
| Excellent | No critical violation was observed during that inspection. |
| Satisfactory | A critical violation was found and corrected during the inspection. |
| Needs Improvement | A critical violation remained uncorrected when the inspection ended. |
| Unsatisfactory | Serious food-safety or sanitation conditions required closure. |
These are qualitative labels rather than points or letter grades. The county says its response can include correction during the visit, follow-up, or enforcement according to severity. The monthly file also uses N/A, which the FAQ leaves undefined, so record N/A as published and ask the county what it means for that row.
Each rating belongs to a dated inspection. Excellent describes that visit only. Satisfactory still records a corrected critical finding. For Needs Improvement, the item, correction date and later verification are in the detailed report. Unsatisfactory records a closure outcome associated with that inspection; the reopening date comes from a later county record.
Requesting a detailed Mohave County inspection report
The Environmental Health program page identifies the responsible county division and lists 928-757-0901. The county Services page links a Public Records route. Confirm the current submission path with Environmental Health before sending a request.
Identify the establishment name, street address, exact Description, inspection date, rating, area heading, and PDF month. Ask for the detailed inspection report and, for the same operation and date range, any later follow-up inspection, correction verification, closure or suspension document, and reopening or reinstatement decision. Also ask the county to identify the facility or permit record tied to the Description. This prevents records for several operations at one premises from being merged.
When the detailed report arrives, use the violation-reading guide to keep the cited requirement, inspector observation, onsite correction, and later verification distinct.
Keep an enforcement order and permission to resume operations as separate dated evidence. The closure and permit suspension guide explains why proof of closure, correction and authorization to reopen are three separate records.
If a row or month you need is missing online, include it in your records request.
