Start with the restaurant’s municipality. The Monmouth County Office of Public Health is one of five health departments in the county and lists the towns it serves. The restaurant inspection page maps authorities to towns. New Jersey’s Retail Food Project covers food facilities on state property and at Turnpike or Garden State Parkway rest stops; route those records there. Use that crosswalk to identify the likely record holder before requesting a missing report or operating-status document.
Search by municipality
Use Municipality; add Establishment Name if needed. Results showed name, address, rating, an unlabeled date, and a View Inspection Report link. Rows had no issuer or facility-history link. Confirm the physical address.
One PDF’s Date differed from its unlabeled result date. Use the PDF Date to describe that inspection. An older report link returned a missing-file response, which does not prove every earlier report is unavailable. If a link fails, keep the row details for a targeted request to the responsible department.
The results included Satisfactory and Incomplete. The page’s rating legend does not define Incomplete. Do not read it as failure, closure, or clearance; ask the issuing department what the entry means.
Read the PDF as a separate record
An opened PDF carried a department heading and Activity Type, Evaluation, Risk Type, and Date fields. It separated Foodborne Illness Risk Factors and Interventions from Good Retail Practices. Numbered N.J.A.C. 8:24 items led to Remarks on a continuation page. Read the remarks with the cited item; a top-line evaluation does not explain the observation.
The form used IN, OUT, N.O., N.A., COS, and R. New Jersey’s current retail-food page links its November 2013 marking instructions. The instructions define IN as compliant, OUT as not compliant, N.O. as not observed, and N.A. as not applicable. COS means all violations cited under that item were corrected and verified before the inspection ended; R marks the same violation under the same item number as on the last inspection. An OUT item remains visible after on-site correction. The site’s violation-reading guide explains why the narrative matters.
An empty Reinspection on or After field does not show that no return was needed or every condition was resolved. Seek a later report or ask the issuer. The opened PDF showed no numeric score to compute from its marks; Risk Type is separate from its evaluation.
| Record or field | Date to use | What it can establish |
|---|---|---|
| Search result | Keep the unlabeled row date as displayed | A lead to a report, not a verified inspection date or full history. |
| Inspection PDF | Use the report’s own Date field | That event’s type, evaluation, item marks, and remarks. |
| Later action | Ask the issuing health authority for its dated record | Whether a reinspection, closure, or authorization to resume was actually documented. |
Match the rating to the later agency decision
The portal legend says Satisfactory reflects substantial compliance. Conditionally Satisfactory calls for a full, unannounced reinspection within a reasonable time based on the violation. Unsatisfactory identifies an imminent hazard: the authority asks the operator to cease voluntarily and may use legal measures to stop food service until reevaluation. These evaluations are not a numeric point scale or proof of current operating status.
The county’s Consumer Public Health program inspects food establishments in its jurisdiction under New Jersey Chapter 24, investigates complaints, and takes enforcement action when needed. Its records statement does not give it custody of every other department’s file. Identify the municipality’s health authority before requesting a missing reinspection or order.
For county-held records, use Monmouth’s OPRA page and request portal. The county directs requests for records it does not maintain to the responsible town or agency. For a local or regional department, use the restaurant page’s contact list and ask for its process. Include the business name, address, municipality, both dates if they differ, and the precise report or action sought. The site’s reinspection guide helps distinguish a scheduled return, completed revisit, and authorization to resume.
Official pages and the report route were checked September 27, 2026. A dated inspection documents conditions recorded then; it does not guarantee present food safety. Recheck the latest available report and any later agency action for the exact location.
